Why roman-numeral at end of lastname in ECFMG?

I see at the top panel of ECFMG:

> USMLE ID: 1-234-567-8 Name: LeonardoV, David Nick

I am at 5th year of my medical studies (10th semester). I think the "V" indicates that. But why it is integrated together with my lastname.
