Do forms 344 and 345 have to be attested by the Dean?

Hey everybody,

I guess the title of this thread is pretty clear as to what my question is. I actually can't find a place for my dean to sign, and I'm wondering if it's just me who's supposed to fill this form, or am I misinterpreting something. Also, where does form 327-A come in? Is it still used?

Any help will be greatly appreciated :)
