transfer credits doubt..

hi, i am an img from india, 1. i completed my 4 yrs and exams from my clg and transferred and completed my internship in another hospital , does this count to transfer of credit or not, i am sure someone here might have the same doubt.....

2.do the transcripts mean only my marksheet or also i have to mention attendance (classes attended per subject)....
..... pls help me out, i am confused...
