Form 186; Which Type I pick?

I had a greencard in US, but now I come back my country for a few reasons. I'm applying for my step 1 in my country. I have a question about this
ECFMG offered me 2 forms 186.
Can I get and finish form 186 only signed by medical school officials ? ( I'm living near my school and I can go and sign in front of them )
Or I have to get form 186 signed by Notary public and medical school officials ?
Which form 186 for my case ?. Thank you everyone
